Inflatable Obstacle Course and Slide, Bouncy Castles, Soccer, Tag, Legos, Coloring and more. 

SATURDAY SPORTS NIGHT (GRADES K - 7)

7pm - 9pm / $15 per child

Inflatables will be deflated and rolled up to give the kids space to play Mini Sticks, Soccer, Volleyball, etc.

Please register below and sign in at the door when you arrive. Payment can be made when you arrive or by EMT to velocityclarenville@gmail.com. Please include your child's name in the memo section so we can properly apply your payment.

RELEASE OF LIABILITY, WAIVER OF CLAIMS,

ASSUMPTION OF RISKS AND INDEMNITY AGREEMENT.

PLEASE READ CAREFULLY.  BY SUBMITTING THIS FORM YOU WAIVE CERTAIN LEGAL RIGHTS (INCLUDING THE RIGHT TO SUE).  In consideration of being permitted to participate in any way in the activities of VELOCITY (Hereinafter "VELOCITY"): I / We understand that there are risks and dangers associated with participation in martial arts or events and activities which could result in bodily injury, partial and/or total disability, paralysis and even death. The social and economic losses and/or damages, which could result from these risks and dangers could be severe. These risks and dangers may be caused by the action, inaction or negligence of the participants or the action, inaction or negligence of others, including, but not limited to, VELOCITY. There may be other risks not known to us or are not reasonably foreseeable at this time. I/We agree to the terms of the waiver, release, covenant not to sue and indemnity agreement as set forth herein below. I/We, hereby both freely and voluntarily give my/our consent and approval for participation in ANY of VELOCITY sports programs or related activities, and further, agree individually, and/or on behalf of my/our child, to the terms of the waiver, release, covenant not to sue and indemnity agreement as set forth herein below. I/We assume all risks and hazards incidental to such participation, including, but not limited to, physical injury and transportation to and from activities; and I/we hereby waive, release, absolve, indemnify and agree to hold harmless VELOCITY, the program coaches, sponsors, supervisors, participants, person(s) transporting me and, VELOCITY members, officers, directors, employees, volunteers, agents or any other representatives of VELOCITY, from and against any and all causes of action, claims, demands, losses, expenses or liability of any nature whatsoever, in law or in equity, arising from the activities contemplated hereby. In the same capacities, I covenant and agree not to sue VELOCITY, the program coaches, sponsors, supervisors, participants, and person transporting me, VELOCITY members, officers, directors, employees, volunteers, agents or any other representatives of VELOCITY, for any such causes of action, claims, demands, losses, expenses or liability. In case of injury or illness, I/We give my/our consent to emergency transportation and the administration of any first aid, medical and/or dental treatment. I/We accept responsibility for the payment of any such emergency transportation and treatment expenses and any related or subsequent medical bills. EACH OF THE UNDERSIGNED further expressly agrees that the foregoing release, waiver, and indemnity agreement is intended to be as broad and inclusive as is permitted by the law of the Province in which the event is conducted and that if any portion is held invalid, it is agreed that the balance shall, notwithstanding continue in full legal force and effect.  I HAVE READ THIS RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME AND INTEND MY SIGNATURE TO BE COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.
