ABOUT VELOCITY MARTIAL ARTS
Velocity Martial Arts classes are co-ed and open to both recreational and competitive students. We offer instruction in a variety of disciplines including Brazilian Jiu Jitsu (Grappling), Kenpo Karate, Kickboxing and Mixed Martial Arts. Classes are available for ages 5 to adult. A typical class includes a warm-up, stretching, fitness, kickboxing and jiu jitsu / grappling techniques and drills, self-defense and live sparring and grappling.

Registration Fees can be paid by e-transfer to email@example.com or can be paid with cash or cheque. Please make sure to note the "Students Name" in the memo/notes section if sending by email transfer or paying by cheque. Payment plan available on request.
COVID 19 Guidelines & Restrictions - We will be following the guidance provided by the Government of Newfoundland & Labrador and the Department of Health and Community Service regarding cleaning and physical distancing. Group size is limited to 20 participants per class. Due to these restrictions, parents and spectators are not permitted to stay during class. Masks are required when entering and exiting the facility.

RELEASE OF LIABILITY, WAIVER OF CLAIMS, ASSUMPTION OF RISKS AND INDEMNITY AGREEMENT. DECLARATION OF COMPLIANCE – COVID-19.
PLEASE READ CAREFULLY. BY SUBMITTING THIS FORM YOU WAIVE CERTAIN LEGAL RIGHTS (INCLUDING THE RIGHT TO SUE). In consideration of being permitted to participate in any way in the activities of VELOCITY (Hereinafter "VELOCITY"): I / We understand that there are risks and dangers associated with participation in martial arts or events and activities which could result in bodily injury, partial and/or total disability, paralysis and even death. The social and economic losses and/or damages, which could result from these risks and dangers could be severe. These risks and dangers may be caused by the action, inaction or negligence of the participants or the action, inaction or negligence of others, including, but not limited to, VELOCITY. There may be other risks not known to us or are not reasonably foreseeable at this time. I/We agree to the terms of the waiver, release, covenant not to sue and indemnity agreement as set forth herein below. I/We, hereby both freely and voluntarily give my/our consent and approval for participation in ANY of VELOCITY sports programs or related activities, and further, agree individually, and/or on behalf of my/our child, to the terms of the waiver, release, covenant not to sue and indemnity agreement as set forth herein below. I/We assume all risks and hazards incidental to such participation, including, but not limited to, physical injury and transportation to and from activities; and I/we hereby waive, release, absolve, indemnify and agree to hold harmless VELOCITY, the program coaches, sponsors, supervisors, participants, person(s) transporting me and, VELOCITY members, officers, directors, employees, volunteers, agents or any other representatives of VELOCITY, from and against any and all causes of action, claims, demands, losses, expenses or liability of any nature whatsoever, in law or in equity, arising from the activities contemplated hereby. In the same capacities, I covenant and agree not to sue VELOCITY, the program coaches, sponsors, supervisors, participants, and person transporting me, VELOCITY members, officers, directors, employees, volunteers, agents or any other representatives of VELOCITY, for any such causes of action, claims, demands, losses, expenses or liability. In case of injury or illness, I/We give my/our consent to emergency transportation and the administration of any first aid, medical and/or dental treatment. I/We accept responsibility for the payment of any such emergency transportation and treatment expenses and any related or subsequent medical bills. EACH OF THE UNDERSIGNED further expressly agrees that the foregoing release, waiver, and indemnity agreement is intended to be as broad and inclusive as is permitted by the law of the Province in which the event is conducted and that if any portion is held invalid, it is agreed that the balance shall, notwithstanding continue in full legal force and effect. I HAVE READ THIS RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT, ASSURANCE, OR GUARANTEE BEING MADE TO ME AND INTEND MY SIGNATURE TO BE COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW.
DECLARATION OF COMPLIANCE – COVID-19
ALL PARTICIPANTS ENTERING THE FACILITY MUST COMPLY WITH THIS DECLARATION
Velocity Sports & Recreation requires the disclosure of exposure or illness is in order to safeguard the health and safety of all participants and limit the further outbreak of COVID-19. This Declaration of Compliance will be kept safely, and personal information will not be disclosed unless as required by law or with your consent.
A participant (or the participant’s parent/guardian, if the participant is younger than 18 years old) who is unable to agree to the terms outlined in this document is not permitted to enter the Organization’s facilities or participate in the Organization’s activities, programs, or services.
I, the undersigned being the participant named above and the participant’s parent/guardian (if the participant is younger than 18 years old), hereby acknowledge and agree to the terms outlined in this document:
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization and COVID-19 is extremely contagious. The Organization has put in place preventative measures to reduce the spread of COVID-19 and requires all participants (or their parent/guardian, when applicable) to adhere to the compliance standards described in this document.
The participant has not been diagnosed with COVID-19. OR If the participant was diagnosed with COVID-19, the participant was cleared as noncontagious by provincial or local public health authorities more than 14 days prior to the date this Declaration of Compliance was signed.
The participant has not been exposed to a person with a confirmed or suspected case of COVID-19. However, if the participant was exposed to a person with a confirmed case of COVID-19, the date of exposure was more than 14 days prior to the date this Declaration of Compliance was signed.
The participant is attending or participating voluntarily and understands the risks associated with COVID-19. The participant (or the participant’s parent/guardian, on behalf of the participant (when applicable) agrees to assume those risks, including but not limited to exposure and being infected.
The participant has not, nor has anyone in the participant’s household, experienced cold or flu-like symptoms in the last 14 days (including fever, cough, sore throat, shortness of breach, respiratory illness, difficulty breathing).
If the participant experiences, or if anyone in the participant’s household experiences, any cold or flu-like symptoms after submitting this Declaration of Compliance, the participant will immediately isolate and not attend any of the Organization’s facilities, activities, programs or services until at least 14 days have passed since those symptoms were last experienced.
The participant has not, nor has any member of the participant’s household, travelled to, or had a lay-over in any country outside Canada, or in any province outside of Newfoundland & Labrador in the past 14 days. If the participant travels, or if anyone in the participant’s household travels, outside the Province of Newfoundland & Labrador after submitting this Declaration of Compliance, the participant will not attend any of the Organization’s facilities, activities, programs or services until at least 14 days have passed since the date of return.
The participant is following recommended guidelines, including but not limited to, practicing physical distancing, trying to maintain separation of six feet from others, frequent handwashing, and otherwise limiting exposure to COVID-19.
The participant will follow the safety, physical distancing, and hygiene protocols of the Organization.
This document will remain in effect until the Organization, per the direction of the provincial government and provincial health officials, determines that the acknowledgements in this Declaration of Compliance are no longer required.
The Organization may remove the participant from the facility or from participation in the activities, programs or services of the Organization at any time and for any reason if the Organization believes, in its sole discretion, that the participant is no longer in compliance with any of the compliance standards described in this document.
